Assessing the Burden of Adult-Onset Still’s Disease on Patients and Healthcare Systems

Adult-onset Still’s disease (AOSD) is found to pose an uncertain burden on patients and healthcare systems. In effort to estimate this burden, a systematic review was conducted to gather data related to health-related quality of life (HRQoL), utilities, costs, and healthcare resource use. The search spanned across twelve databases, four conferences, and three major technology assessment and regulatory agency websites in August 2022.

The review also included reference lists of systematic reviews published since 2017. Out of the data gathered 16 studies were found to be pertinent for inclusion. Among these, eight studies reported on HRQoL outcomes, with one of these studies also presenting utility data. Furthermore, two studies reported direct cost outcomes, while seven studies reported data on healthcare resource use. However, no indirect costs were identified in the review.

The outcomes varied, thus inhibiting the comparability of results across studies. The SF-36 data, which measures patient health, showed impairments in AOSD on most scales, particularly those concerning physical activity. Patients with active AOSD showed lower mean SF-36 data across all subscales compared to patients with inactive AOSD. Biologic therapy was found to improve the SF-36 physical health summary.

In a single study, utility scores were significantly lower for AOSD patients compared to healthy controls. Although limited, the direct economic costs data that were identified were substantial. The length of hospital stay for AOSD patients ranged from 6.1 to 23.5 days. This systematic review clearly emphasizes the scarcity of research reporting the HRQoL and cost burden of AOSD.
