In a groundbreaking study, researchers have conducted a comprehensive analysis comparing the effectiveness and safety of various pharmacological interventions for adults suffering from overweight and obesity. This systematic review and network meta-analysis shines a light on the efficacy of weight-lowering drugs, offering a beacon of hope for those whose lifestyle modifications have failed to yield the desired results. The study meticulously compiled data from numerous sources, including PubMed, Embase, and the Cochrane Library, up until March 23, 2021, focusing on randomized controlled trials that scrutinize the impact of these drugs on body weight.

The extensive search culminated in the identification of 132 eligible trials involving 48,209 participants. The evidence presented demonstrates that all considered drugs were effective in reducing body weight when compared to lifestyle modifications alone. Notably, phentermine-topiramate emerged as the most potent, followed closely by GLP-1 receptor agonists.

Certain drugs, including naltrexone-bupropion, phentermine-topiramate, GLP-1 receptor agonists, and orlistat, were associated with an increased incidence of adverse events leading to discontinuation. A standout finding was the remarkable efficacy of semaglutide, a GLP-1 receptor agonist, which outperformed other drugs in terms of weight reduction while maintaining a comparable risk profile regarding adverse events.

The implications of these findings are significant, offering a refined understanding of the pharmacological landscape for managing obesity. Among the array of GLP-1 agonists, semaglutide is particularly promising, presenting a potent option for significant weight reduction. This research was supported by the 1.3.5 Project for Disciplines of Excellence at the West China Hospital, Sichuan University, underscoring the collaborative effort in advancing obesity treatment.
